Losing lots of hair daily, itchy scalp, dandruff, etc.?

I'm a 17 year old male. Long black thick hair. For the past year or so, i've noticed i've been losing a lot of hair whenever I run my hand through it or when im in the shower. It hasn't become thin or visibly different. I never comb it due to it being so damn thick and the knots, don't get me started on the knots. But when I do try to comb it, the extreme loss of hair is frightening. I also have an itchy scalp and have dandruff flakes. Any suggestions to what may be the problem? I'm thinking it may be my diet. Should I start taking a multi-vitamin? Also, whats a good shampoo for combatting the itchy and flakey scalp?
What kind of comb should I use? And should I be using conditioner and such?
